The Skokie Health and Human Services Department is offering Moderna COVID-19 vaccine at this clinic to the following people:

-1st dose
- 2nd doses for people vaccinated with their 1st dose prior to May 27

RESIDENTS MUST BE 18 OR OVER TO REGISTER FOR THIS CLINIC
