Memorial Hospital is currently vaccinating Illinois residents or those who work in Illinois but reside in another state.

On the day of your appointment, please arrive approximately 5-minutes prior to your scheduled appointment. Unless physical assistance is required, please enter the facility by yourself. This will assist in managing social distance requirements and maintain an efficient workflow. As a reminder, appointments are required, and walk-ins are not accepted.
