8/17/21 - Woodford County - WCHD Walk In - Moderna, Pfizer, or J&J
This walk-in clinic will be held from 9:00 AM to 11:00 AM.
You must be 12 years of age or older. Minors must be accompanied by parent or legal guardian.
You must bring your CDC Vaccination Card if this is your second dose.
You are NOT Eligible to receive a vaccine at this clinic if you:
--Received convalescent plasma or antibody therapy in the last 90 days
--Have had a severe reaction to a vaccine (anaphylaxis)
IMPORTANT: If you pre-register, please print and bring barcode or be able to show it on your device when you come to the clinic.
