Please arrive to your appointment on time with your completed consent form, a short sleeve shirt on, and your I.D. You can print a consent at www.cchd.net or you can pick up a copy from the pickup box outside the front door of the health department. If this is your second dose please bring the vaccine record card you received at your first appointment.
