This clinic is for eligible Phase 1A and 1B individuals ONLY.

As of now, Woodford County is NOT able to vaccinate individuals that are eligible in the Phase 1B Expansion (16-64 with preexisting conditions) due to limited vaccine availability.

IMPORTANT: Please arrive on time. If you are more than 10 minutes late to your appointment, your spot will be filled.

You are NOT Eligible to receive a vaccine at this clinic if you:
--Received any vaccine in the last 14 days
--Received convalescent plasma or antibody therapy in the last 90 days
--Have had a severe reaction to a vaccine (anaphylaxis)
