Swedish Hospital is cohosting a vaccine clinic with the offices of the 39th Ward Alderperson Samantha Nugent and State Senator Ram Villivalam for the North Park community. This event will take place at the Albany Park Library on December 15th 2021.

The Pfizer vaccine will be available for people ages 5 and up. Anyone under the age of 18 must be accompanied by their parent or guardian and show proof of identification.

The J&J single dose vaccine is available for people ages 18 and up.

Booster doses will be available to anyone who received their second dose 6+ months ago.

Please bring a photo ID and vaccination card if available.
