Who Qualifies for a Booster shot of Pfizer COVID-19 Vaccine:
The CDC recommends anyone 12 years and older get a COVID-19 vaccine booster dose five months after receiving their second Moderna, five months after receiving their second dose of Pfizer-BioNTech vaccine, or two months after receiving the one-dose Johnson & Johnson vaccine.

Who Qualifies for a 3rd Dose of Pfizer COVID-19 Vaccine:
Currently, CDC is recommending that moderately to severely immunocompromised people, aged 5 years and older, receive an additional dose. For people ages 5-11 years old, please click here https://events.juvare.com/IL-IDPH/rnmyw/ to schedule an appointment. For people ages 12 and up, please stay on this page to schedule your appointment. This includes people who have:
• Been receiving active cancer treatment for tumors or cancers of the blood
• Received an organ transplant and are taking medicine to suppress the immune system
• Received a stem cell transplant within the last 2 years or are taking medicine to suppress the immune system
• Moderate or severe primary immunodeficiency (such as DiGeorge syndrome, Wiskott-Aldrich syndrome)
• Advanced or untreated HIV infection
• Active treatment with high-dose corticosteroids or other drugs that may suppress your immune response
If your condition or medication isn’t on this list and you think you have a weakened immune system, talk to your healthcare provider about your
medical condition and whether getting an additional dose is appropriate.

Your appointment is at the Will County Health Department, 501 Ella Ave, Joliet, IL 60433

-Arrive at your scheduled time, not before.
-Masks must be worn at all times.
-All individuals will be screened for symptoms and will be rescheduled if not well enough to receive vaccine at the time of their appointment.
-Please allow 30 minutes for your appointment.
-Make sure to wear a short sleeve shirt (we will be vaccinating the upper arm)
