Please arrive to your appointment on time with your completed consent form, a short sleeve shirt on,
and your I.D. You can print a consent at www.cchd.net or you can pick up a copy from the pickup box
outside the front door of the health department. All minors must be accompanied by their parent/guardian.
If this is your second dose please bring the vaccine record card you received at your first appointment.
