This is a second dose Moderna Vaccine Drive Thru Clinic for individuals 18 years old and older. Your first dose of Moderna vaccine has to have been on or before June 3, 2021. This is a drive thru clinic, you will remain in your care the entire time.

Please bring your first dose shot record card so we can verify you are eligible for your second dose on this day.

• If you are pregnant, you must bring a script from your doctor to receive the vaccine.
• To save time, please go to www.effcohealth.org to print and fill out the vaccine consent form prior to coming to the clinic to get your COVID vaccination.
• Please bring a photo ID with you.
• If you have insurance, please bring your insurance card. We may bill an administration fee to your insurance and that will be accepted as full payment, there will be NO CHARGE to the individual.
• Wear short sleeves
• Bring your own pen.
• Please bring your pre-printed ticket, completed and signed consent form, your shot record card and insurance card to your appointment.
You will be asked to stay for 15 minutes of observation after you receive the vaccine.
Final Directions
The clinic entrance is at the North East corner of the parking lot of the Effingham County Health Department.
There are no bathroom facilities available.
