8/24/21 - Woodford County - WCHD Walk-in Clinic - Pfizer, Moderna, or J&J

This walk-in clinic will be held from 9:00 AM - 11:00 AM. You may come in anytime during that timeframe. Your confirmation ticket may show an exact appointment time, but know you may come anytime during the clinic.

You must be 18 years or older to receive Moderna or J&J vaccines. You must be 12 years of age or older to receive Pfizer vaccine.

A parent or legal guardian must accompany any minor.

You must bring your CDC Vaccination Card if this is your second dose.

You are NOT Eligible to receive a vaccine at this clinic if you:
--Received convalescent plasma or antibody therapy in the last 90 days
--Have had a severe reaction to a vaccine (anaphylaxis)

IMPORTANT: If you pre-register, please print and bring barcode or be able to show it on your device when you come to the clinic.
