This is a drive thru clinic for Third Dose Moderna and Third/Booster Dose Pfizer Vaccine. You must receive the same vaccine on the third dose as you did on the first two doses. Please make sure you make an appointment for the appropriate vaccine. This clinic is a drive thru clinic, you will remain in your car the entire time. You must meet the following requirements to receive a third/booster Covid vaccine.

Moderna-Third Dose
Must be immunocompromised
Must be 18 years old or older
Second dose of Moderna was received on or before October 1, 2021

Pfizer-Third Dose
Must be immunocompromised
Must be 12 years old or older
Second dose of Pfizer was received on or before October 1, 2021

Pfizer-Booster Dose
People 65 years or older
Residents 18 years or older in long-term care settings
People 50-64 with underlying medical conditions
Second dose of Pfizer was received on or before April 29, 2021

• To save time, please go to www.effcohealth.org to print and fill out the vaccine consent form prior to coming to the clinic to get your COVID vaccination.
• Please bring a photo ID with you.
• If you have insurance, please bring your insurance card. We may bill an administration fee to your insurance and that will be accepted as full payment, there will be NO CHARGE to the individual.
• Wear short sleeves.
• Bring your own pen.
• Please bring your pre-printed ticket, completed and signed consent form, and insurance card to your appointment.
• Please bring your vaccine record card with you.

You will be asked to stay for 15 minutes of observation after you receive the vaccine.

Final Directions
The clinic entrance is at the North East corner of the parking lot of the Effingham County Health Department.
There are no bathroom facilities available.
Please bring your pre-printed ticket, completed and signed consent form, and insurance card to your appointment.
Please bring your vaccines record card with you.
