05/13/21 Effingham County Health Department First and Second Dose Moderna Drive Thru Clinic
This is a First and Second Dose Moderna Vaccine Drive Thru Clinic. This clinic is for all Illinois residents 18 and older. If you are making a second dose appointment, your first dose of vaccine has to have been on or before April 15, 2021. This clinic is a drive thru clinic, you will remain in your car the entire time.
If you are pregnant, you must bring a script from your doctor to receive the vaccine.
• To save time, please go to www.effcohealth.org to print and fill out the vaccine consent form prior to coming to the clinic to get your COVID vaccination.
• Please bring a photo ID with you.
• If you have insurance, please bring your insurance card. We may bill an administration fee to your insurance and that will be accepted as full payment, there will be NO CHARGE to the individual.
• Wear short sleeves
• Bring your own pen.
• Please bring your pre-printed ticket, completed and signed consent form, and insurance card to your appointment.
• If this is your second dose, please bring your vaccine record card you were given for your first dose.
You will be asked to stay for 15 minutes of observation after you receive the vaccine.
The clinic entrance is at the North-East corner of the parking lot at the corner of Virginia and Mulberry Streets, across from HSHS St. Anthony's Hospital.
There are no bathroom facilities available.
