04/14 - Pulaski County – Shawnee Community College - MODERNA Dose #1 & 2
This is a Moderna two-dose vaccine option to be given at the Shawnee Community College Main Campus (8364 Shawnee College Rd., Ullin, IL 62992). Please follow Southern 7 signage for parking and entrance into the vaccination area. Please have a mask on when entering the building. Must be 18 years old or older and either a resident of Illinois or have proof of employment in the state of Illinois. Make sure to bring Driver's License or Photo ID and/or proof of employment if applicable. If this is your second dose, please bring your vaccine record card as well.
