Please wear a mask, bring a valid ID, your VOUCHER BARCODE and your Vaccine Card (if applicable) to receive your vaccine.
*If you are receiving a first dose COVID-19 vaccine at this clinic, we will not be back onsite at the schools to provide second doses. You will need to schedule the second dose at the Maple Street Clinic, 109 East Maple, Gillespie or at any other COVID-19 Vaccine Provider site.
