Appointment times are expected to take 45 minutes, including the observation period of 15 minutes after vaccination.

Second doses available on this date are for persons who received their first dose on June 4th or before. You must bring a copy of your first vaccine with to this appointment.

First doses received on this date will need to seek out a second dose 3-6 weeks later, either through a local pharmacy or through the Health Department is supplies are available. There is not a mass vaccination clinic planned for second doses.

Appointment are created to allow for appropriate distancing; therefore no one will be allowed in to the clinic more than 15 min prior to appointment time.
Persons with a history of adverse reactions to vaccine/medication should not schedule for this clinic. Rather, call 815-941-3404 to inquire about special accommodations.
Please wear clothing that provides easy access to your upper arm.
