Who Qualifies for a 3rd Dose of Pfizer COVID-19 Vaccine:
Currently, CDC is recommending that moderately to severely immunocompromised people aged 5+ can receive an additional dose. This clinic is for those 12 years and older. If the person is aged 5-11, they can receive 3rd dose at our Joliet Office, 501 Ella Ave, Joliet, IL 60433. • Been receiving active cancer treatment for tumors or cancers of the blood
• Received an organ transplant and are taking medicine to suppress the immune system
• Received a stem cell transplant within the last 2 years or are taking medicine to suppress the immune system
• Moderate or severe primary immunodeficiency (such as DiGeorge syndrome, Wiskott-Aldrich syndrome)
• Advanced or untreated HIV infection
• Active treatment with high-dose corticosteroids or other drugs that may suppress your immune response
If your condition or medication isn’t on this list and you think you have a weakened immune system, talk to your healthcare provider about your
medical condition and whether getting an additional dose is appropriate.

Who Qualifies for a Booster shot of Pfizer COVID-19 Vaccine:
The CDC recommends anyone 12 years and older get a COVID-19 vaccine booster dose five months after receiving their second Moderna or five months after receiving Pfizer-BioNTech vaccine, or two months after receiving the one-dose Johnson & Johnson vaccine.

Your appointment is at the Will County Health Department-Bolingbrook Branch, 323 Quadrangle Drive, Bolingbrook, IL 60440

-Arrive at your scheduled time, not before.
-Masks must be worn at all times.
-All individuals will be screened for symptoms and will be rescheduled if not well enough to receive vaccine at the time of their appointment.
-Please allow 30 minutes for your appointment.
-Make sure to wear a short sleeve shirt (we will be vaccinating the upper arm)
