The Skokie Health and Human Services Department is offering:
PRIMARY SERIES: FIRST and SECOND doses ages 12 years +.
THIRD FULL DOSE for immune compromised individuals ages 12 years +.
BIVALENT BOOSETER dose 2 months after your last primary series dose for ages 6 years +.
To be considered up-to-date: MUST have primary series AND bivalent booster dose completed.
Primary Series: Second dose is administered 28 days after first dose. IF immune compromised, third dose is administered 4 weeks after second dose.
Bivalent Booster dose (for NON-immunocompromised peoples) is administered 2 months after second dose.
Bivalent Booster dose (for immunocompromised peoples) is administered 2 months after third dose.
Registrants will receive a confirmation email.
Please bring your Covid-19 Vaccination Record Card with you.
Please show up for your appointment no more than 5 minutes prior to your scheduled appointment time.
New appointment times will open the 4th Wednesday of the month at 9 a.m.
