This is an appointment for a COVID-19 vaccine to be given at the Southern 7 Health Department's Massac County clinic office (1230 Commercial Park Rd.) in Metropolis, IL. Please follow Southern 7 signage for parking and entrance into the vaccination area. Please have a mask on when entering the building. Must be 18 years old or older and either a resident of Illinois or have proof of employment in the state of Illinois. Make sure to bring Driver's License or Photo ID and/or proof of employment if applicable. If this is your second dose, please bring your vaccine record card as well.
