Minors (16 & 17 year old) must be accompanied by a parent or legal guardian.

Please bring a copy of the ticket with you - paper copy or on your phone. We will have paper copies available at the clinic for those unable to print. Your Dose 2 appointment will be scheduled after you receive Dose 1.

Appointments are created to allow for appropriate distancing; therefore no one will be allowed in to the clinic more than 15 min prior to appointment time.
