Illinois Department of Human Services is hosting ongoing vaccination at our Chicago-Read Mental Health Center Site. All those attending will be required to wear a mask and adhere to site screening protocols. 

This event will be located in the "K building" of the facility.

IDHS is offering the Moderna Vaccine which requires 2 doses. After scheduling your first dose, we encourage you to go back in and schedule your second dose using the same link. Your second dose should be scheduled on or around 28 days after your first vaccination.
