Only 1 appointment can be made per patient.
If multiple patients require a COVID-19 test, a separate appointment will be required for each.
*Please note, a nasal swab PCR Lab Test REQUIRES PROOF OF VALID HEALTH INSURANCE to be uploaded during the registration process or brought to the testing appointment for scanning.
