Please bring your ticket, signed consent form and your insurance card (please let us know if your insurance has changed from your first vaccination) and your COVID-19 Vaccination Card (if this will be your 2nd dose). If you have Medicare, bring your new red/white/blue card (the new card should not have a social security # on it).
Where does the event happen? Exit off of Hwy 130 onto Clayton Avenue. Go to stop sign and turn left onto Edwards Street. Follow signs for drive through clinic.
