You must be 18 years or older.

You must bring your CDC Vaccination Card if this is your second dose.

You are NOT Eligible to receive a vaccine at this clinic if you:
--Received convalescent plasma or antibody therapy in the last 90 days
--Have had a severe reaction to a vaccine (anaphylaxis)

IMPORTANT: If you preregister, please print barcode or be able to show it on your device when you come to the clinic. If you choose not to preregister, please bring state issued ID.

This is a walk-in clinic. You may come anytime between 9:00 AM-Noon.
