The Skokie Health and Human Services Department is offering SECOND DOSES of the Pfizer COVID-19 vaccine at this clinic only to the following people:

Persons vaccinated prior to 06/04/21.

RESIDENTS MUST BE 12 OR OVER TO REGISTER FOR THIS CLINIC

Registrants will receive a confirmation email. Please print the ticket document attached to the confirmation email and bring it with you. Having a ticket with you will help save time and ensure we can quickly check you in!
