04/13 - Union County - Main Street Center (Tuesday) MODERNA Dose #1 & 2
This is a Moderna two-dose vaccine option to be given at the Main Street Center (400 South Main Street, Anna, IL 62906). Please follow Southern 7 signage for parking and entrance into the vaccination area. Please have a mask on when entering the building. Must be 18 years old or older and either a resident of Illinois or have proof of employment in the state of Illinois. Make sure to bring Driver's License or Photo ID and/or proof of employment if applicable. If this is your second dose, please bring your vaccine record card as well.
