This clinic is open to all residents of the State of Illinois This clinic is a drive thru clinic. You will remain in your car the entire time. Please bring a driver’s license or ID showing you are resident of the State of Illinois. If you are pregnant, you must bring a script from your doctor to receive the vaccine. 

To save time, please go to www.effcohealth.org to print and fill out the vaccine consent form prior to coming to the clinic to get your COVID vaccination.

Please bring your insurance card. We may bill an administration fee to your insurance and that will be accepted as full payment, there will be NO CHARGE to the individual.

Bring your own pen.

You will be asked to stay for 15 minutes of observation after you receive the vaccine.

Please bring your pre-printed ticket, completed and signed consent form, ID, and insurance card to your appointment.
