This is a First Dose Pfizer Vaccine Clinic. This clinic is individuals 12-17 years old. The clinic is located at the Effingham County Health Department, 901 W. Virginia, Effingham, IL. Once you arrive at the department, please remain in your car and call 217-342-9237. A staff member will come outside and get you when we are ready for your vaccination. An appointment for your second dose of vaccine will be made at your first dose appointment.
Masks are required for anyone entering the building. No one will be admitted without one.
If you are pregnant, you must bring a script from your doctor to receive the vaccine.
• To save time, please go to www.effcohealth.org to print and fill out the vaccine consent form prior to coming to the clinic to get your
COVID vaccination.
• Please bring a photo ID with you.
• If you have insurance, please bring your insurance card. We may bill an administration fee to your insurance and that will be accepted as
full payment, there will be NO CHARGE to the individual.
• Wear short sleeves
• Bring your own pen.
• Please bring your pre-printed ticket, completed and signed consent form, and insurance card to your appointment.

You will be asked to stay for 15 minutes of observation after you receive the vaccine.
