Community Vaccine Alliance Vaccination Clinic is located at Joan Dachs Bais Yaakov Elementary School, 3200 W Peterson Avenue, Chicago, IL 60659. Please enter through the Lincoln Avenue entrance.

You must complete the registration in its entirety to book an appointment.

Upon completion, you will receive a confirmation ticket with a unique QR code. You will need to present the ticket to enter the clinic. Please have the ticket available on your phone or bring a printed copy.

This registration is for first does Moderna Vaccine. Once you book in the system, you'll be contacted close to the 4 week date to book your 2nd appointment. You must receive the first and second dose at the same location.
