Chicago Public Schools is excited to welcome its students, their families, and members of the surrounding community to receive the COVID-19 and Influenza vaccines! All Chicagoans aged 6 months and older are eligible to get vaccinated to protect against COVID-19. The COVID-19 vaccine is free, and no proof of insurance nor citizenship is required to get vaccinated. Insurance information is needed for the influenza vaccine (private or public-Medicaid/Medicare)

Appointments are encouraged. Walk-ins are permitted while supplies last.

Eligible patients (minors under the age of 18 years) must be consented by a parent/guardian unless an exception applies and/or written consent has been provided through this registration process.
