OVERVIEW
This is a Moderna First and Second Dose Drive Thru Clinic. Moderna First and Second Dose vaccines will be the only vaccine given at this clinic. Please make sure you make an appointment for the appropriate vaccine. This clinic is a drive thru clinic, you will remain in your car the entire time.
MODERNA FIRST DOSE
Must be 18 years old or older

MODERNA SECOND DOSE
Must be 18 years old or older
First dose of Moderna vaccine has to have been on or before January 27, 2022.

TO SAVE TIME
Please go to www.effcohealth.org to print and fill out the vaccine consent form prior to coming to the clinic to get your COVID vaccination.
Please bring a photo ID with you.
If you have insurance, please bring your insurance card. We may bill an administration fee to your insurance and that will be accepted as full payment, there will be NO CHARGE to the individual.
Wear short sleeves.
Bring your own pen.
Please bring your pre-printed ticket, completed and signed consent form, and insurance card to your appointment.
Please bring your vaccine record card with you.

You will be asked to stay for 15 minutes of observation after you receive the vaccine.

FINAL DIRECTIONS
The clinic entrance is at the North East corner of the parking lot of the Effingham County Health Department.
There are no bathroom facilities available.
Please bring your pre-printed ticket, completed and signed consent form, and insurance card to your appointment.
Please bring your vaccines record card with you.
