1/5/22 Effingham County Health Department Pfizer 1,2,3, and Booster Dose Drive Thru Clinic
OVERVIEW
This is a Pfizer First, Second, Third and Booster Dose Drive Thru Clinic. Pfizer vaccine will be the only vaccine given at this clinic. Please make sure you make an appointment for the appropriate vaccine. This clinic is a drive thru clinic, you will remain in your car the entire time. You must meet the following requirements to receive a third or booster dose of Covid vaccine.
PFIZER FIRST DOSE
Must be 12 years old or older
PFIZER SECOND DOSE
Must be 12 years old or older
First dose of Pfizer was received on or before December 15, 2021
PFIZER THIRD DOSE
Must be 18 years old or older
Second dose of Pfizer was received on or before December 8, 2021
Third Dose is for immunocompromised persons. The conditions that cause moderate to severe immunodeficiency include but are not limited to:
• Active treatment for solid tumor and hematologic malignancies
• Receipt of solid-organ transplant and taking immunosuppressive therapy
• Receipt of CAR*-T-cell or hematopoietic stem cell transplant (within 2 yrs of transplantation or taking immunosuppression therapy)
• Moderate or severe immunodeficiency (e.g., DiGeorge, Wiskott-Aldrich syndrome)
• Advanced or untreated HIV infection
• Active treatment with high-dose corticosterioids (i.e., > 20 mg prednisone or equivalent per day), alkylating agents, antimetabolites, transplant-related immunosuppressive drugs, cancer chemotherapeutic agents classified as severely immunosuppressive, tumor-necrosis (TNF) blockers, and other biologic agents that are immunosuppressive or immunomodulatory
PFIZER BOOSTER DOSE
Must be 18 years old or older
Second Dose of Pfizer must have been on or before July 7, 2021.
• The clinical considerations for heterologous mixing (mix & match) are as follows:
• The same product that was used for the primary regimen should be used for the booster. If that is not available or another product is preferred, heterologous boosting with a single dose of any of the authorized COVID-19 vaccine boosters is acceptable
• Heterologous dosing may be considered for the booster dose only.
TO SAVE TIME
Please go to www.effcohealth.org to print and fill out the vaccine consent form prior to coming to the clinic to get your COVID vaccination.
Please bring a photo ID with you.
If you have insurance, please bring your insurance card. We may bill an administration fee to your insurance and that will be accepted as full payment, there will be NO CHARGE to the individual.
Wear short sleeves.
Bring your own pen.
Please bring your pre-printed ticket, completed and signed consent form, and insurance card to your appointment.
Please bring your vaccine record card with you.
You will be asked to stay for 15 minutes of observation after you receive the vaccine.
FINAL DIRECTIONS
The clinic entrance is at the North East corner of the parking lot of the Effingham County Health Department.
There are no bathroom facilities available.
Please bring your pre-printed ticket, completed and signed consent form, and insurance card to your appointment.
Please bring your vaccines record card with you.
PLEASE DO NOT COME EARLY FOR YOUR APPOINTMENT.
