If you received your first dose of Moderna vaccine from the Jersey County Health Department on January 29, 2021 please sign up here for your second dose. Please arrive no earlier than 5 minutes prior to your appointment time. Please bring your COVID-19 vaccination card and your insurance card. Please print and fill out a copy of the COVID-19 Vaccine Consent form from our website, facebook page, or stop by the Health Department to pick one up prior to the clinic.
