OVERVIEW
This is a Pfizer First Dose Pediatric Drive Thru Clinic. Pfizer First Dose vaccines to 5–18-year-olds will be the only vaccine given at this clinic. Please make sure you make an appointment for the appropriate vaccine. This clinic is a drive thru clinic, you will remain in your car the entire time.

Must be 5-18 years old
PFIZER SECOND DOSE-PEDIATRIC
Must be 5-18
First Dose of Pfizer must have been on or before February 22, 2022.

TO SAVE TIME
Please go to www.effcohealth.org to print and fill out the vaccine consent form prior to coming to the clinic to get your COVID vaccination.
Please bring a photo ID with you.
If you have insurance, please bring your insurance card and a copy of parent/guardian’s ID. We may bill an administration fee to your insurance and that will be accepted as full payment, there will be NO CHARGE to the individual.
Wear short sleeves.
Bring your own pen.
Please bring your pre-printed ticket, completed and signed consent form, and insurance card to your appointment.

You will be asked to stay for 15 minutes of observation after you receive the vaccine.

FINAL DIRECTIONS
The clinic entrance is at the North East corner of the parking lot of the Effingham County Health Department.
There are no bathroom facilities available.
Please bring your pre-printed ticket, completed and signed consent form, and insurance card to your appointment.
