The Skokie Health and Human Services Department is offering FIRST, SECOND, THIRD FULL DOSES & FIRST AND SECOND BOOSTER DOSES of the Pfizer COVID-19 vaccine.

To be considered up-to-date: MUST have primary series AND first booster dose completed.

Primary Series: Second dose is administered 21 days after first dose. IF immune compromised, third dose is administered 4 weeks after second dose.
First Booster (for NON-immunocompromised peoples) is administered 5 months after second dose.
First Booster (for immunocompromised peoples) is administered 3 months after third dose.

Second Booster (for immunocompromised individuals AND individuals ages 50+) is administered 4 months after first booster dose.

RESIDENTS MUST BE 12y.o. OR OVER TO REGISTER FOR THIS CLINIC

Registrants will receive a confirmation email. Please print the ticket document attached to the confirmation email and bring it with you.
Having a ticket with you will help save time and ensure we can quickly check you in!

Please bring your Covid-19 Vaccination Record Card with you.

Please show up for your appointment no more than 5 minutes prior to your scheduled appointment time.

Appointments for the following month will open the last week of the current month.
