You are NOT Eligible to receive a vaccine at this clinic if you:
--Received convalescent plasma or antibody therapy in the last 90 days
--Have had a severe reaction to a vaccine (anaphylaxis)

For individuals who received a Moderna COVID-19 vaccine, the following groups are eligible for a booster shot at 6 months or more after their 2nd Dose:
• 65 years and older
• Age 18+ who live in long-term care settings
• Age 18+ who have the following underlying medical conditions:
o Cancer
o Chronic kidney disease
o Chronic liver disease
o Chronic lung diseases
o Dementia or other neurological conditions
o Diabetes
o Down syndrome
o Heart Conditions
o HIV infection
o Weakened immune system
o Mental health conditions
o Overweight and obesity
o Pregnancy
o Sickle cell disease or thalassemia
o Smoking (current or former)
o Solid organ or blood stem cell transplant
o Stroke or cerebrovascular disease
o Substance use disorder,
o Tuberculosis (TB)
• Age 18+ who work or live in high-risk settings such as:
o First responders (e.g., healthcare workers, firefighters, police, congregate care staff)
o Education staff (e.g., teachers, support staff, daycare workers)
o Food and agriculture workers
o Manufacturing workers
o Corrections workers
o U.S. Postal Service workers
o Public transit workers
o Grocery store workers

IF YOU DO NOT MEET ANY OF THE ABOVE CRITERIA, DO NOT SCHEDULE AN APPOINTMENT AT THIS TIME
