This is a first dose Moderna vaccine clinic. This clinic is open to all State of Illinois residents 18 years and older. This clinic is a drive thru clinic, you will remain in your car the entire time. 

• If you are pregnant, you must bring a script from your doctor to receive the vaccine.
• To save time, please go to www.effcohealth.org to print and fill out the vaccine consent form prior to coming to the clinic to get your COVID vaccination.
• Please bring a photo ID with you.
• If you have insurance, please bring your insurance card. We may bill an administration fee to your insurance and that will be accepted as full payment, there will be NO CHARGE to the individual.
• Wear short sleeves
• Bring your own pen.
• Please bring your pre-printed ticket, completed and signed consent form, and insurance card to your appointment.

You will be asked to stay for 15 minutes of observation after you receive the vaccine.
